Graduation Verification for OPT

This form is for Viterbi Master’s students requesting academic verification of graduation for your OIS OPT form. Please complete this request form to obtain verification from VASE. Upon approval, you will receive a link to your document to upload to the OIS OPT portal. Please review these important notes:

If you are graduating this semester, you should have completed your Application for Graduation with your academic department and your STARS should indicate your graduation date. Your request for OPT academic verification will be denied if your STARS does not indicate your graduation date. You can check your STARS via OASIS when you log-in to myUSC.
If you are enrolled in less than 8 units this semester, you must submit your request for Reduced Course Load (RCL). If you have not done so, please complete the online RCL form as soon as possible.

Statement Of Understanding To Be Read And Signed By The Student

I have read and understood the OPT Instructions and requirements provided by OIS. I acknowledge that:

If USCIS receives my OPT application more than 30 days after the OPT I-20 issuance date, it will be denied. I will check the Data Issued under School Attestation on Page 1 of my OPT I-20 before mailing my application to USCIS to ensure I do not miss the deadline.

If my application is returned or rejected by USCIS for any reason, or if I receive a Request for Evidence (RFE) or a denial notice, I will not resend the application without first checking with OIS.

Once on approved OPT, I must report all employment through the SEVP Portal in order to maintain F-1 status. Failure to report employment could result in termination of my SEVIS record by USCIS.
